Well Visits

To keep your children healthy, it is important to bring them in for regular checkups. We see patients from birth to age 21 years. The list below is our recommended schedule for well child visits. Information about immunizations given during well visits can be found under the "Medical Information" tab on the website.

1 Week, 1 Month, 2 Months, 4 Months, 6 Months, 9 Months, 12 Months, 15 Months, 18 Months, 2 Years, 2 1/2 years, 3 Years, 4 Years, 5 Years, 6 Years, 7 Years, 8 Years, 9 Years, 10 Years, 11 Years, 12 Years, 13 Years, 14 Years, 15 Years, 16 Years, 17 Years, 18 Years, 19 years, 20 years, 21 years.

Parents with children over age 2 years will be asked to complete a "Healthy Habits Survey" at each well visit. We also have information for visits between 12 months and 4 years to help you formulate your questions prior to coming in. You may view and complete this prior to the visit by downloading the age appropriate surveys found in our well child section under the Medical Info tab.
 

Transitioning to an Adult

Many of our high school age patients wonder how long they should see a pediatrician. We offer to take care of our patients through college. In some cases, our patients will transition earlier, and some later. Please see the attached documents for more information. The surveys are tools to stimulate conversation about how to make sure your child is knowledgeable about their health care prior to graduation. We also have care coordinators that can help with the transition process.
